# Best ETQ Reliance (Octave Reliance) alternatives for medical device companies (2026)

ETQ Reliance, now sold as Octave Reliance, is a configurable enterprise QMS used across many industries. Device teams usually start looking elsewhere because of what it leaves out for product development, or because they can't see prices and terms before talking to sales. Here's what our evidence shows, which alternatives fit which situation, and when staying is the better call.

## Why teams look for ETQ Reliance alternatives

**No design controls or software tooling.** There's no design-controls app; Avanos built its own design history file app on the platform.[^etq-avanos] The new product introduction set has APQP and FMEA, but we found no design-input-to-verification traceability, no ISO 14971 risk management file and no design history file export.[^etq-npi] We found no IEC 62304 features or Jira and Git integrations either.[^etq-release][^etq-apps]

**Prices and terms you can't see.** Octave doesn't publish prices. Its FAQ says pricing is "subscription-based (by users, modules or sites)".[^etq-reliance] The subscription agreement auto-renews and can't be cancelled for convenience. It lets fees rise by up to 7% a year, or by a different amount if Octave quotes new pricing at least 30 days before renewal.[^etq-msa] Three reviewers (2021 to 2023) report that things once included became paid.[^etq-capterra][^etq-sa-reviews][^etq-g2-billable] Capterra reviewers call it expensive.[^etq-capterra]

**No published hosting region.** Octave says Reliance runs on AWS but names no region.[^etq-reliance] The EU data processing addendum lets Octave process data wherever it or its subcontractors have facilities.[^etq-dpa] The archived ISO 27001 certificate for the ETQ platform expired on 14 May 2026, and we found no renewal.[^etq-iso]

**A learning curve for new users.** Capterra reviewers are split. Some call it easy to use, while others report that new users find it unintuitive or daunting. Two medical device reviewers found it dated or slower after an upgrade.[^etq-capterra]

**Post-market gaps.** We found no EU vigilance, EUDAMED, UDI or post-market surveillance features.[^etq-lsc]

**Hard to try.** There's no free trial. Octave has a four-minute recorded demo video, and live demos go through sales.[^etq-capterra-profile][^etq-demo] Setup help for Reliance Go, the small-business package, is a paid add-on.[^etq-go]

## Which alternative fits your situation

### You need design controls and risk in the QMS: Greenlight Guru

[Greenlight Guru](/reviews/greenlight-guru/) builds quality software only for medical device companies.[^gg-pricing] Its product tour shows a design control matrix that links user needs, inputs, outputs, verification and validation, next to a risk matrix of hazards, hazardous situations and harms.[^gg-tour] Trace graphs flag missing links across design, risk and tests.[^gg-pd] Each release ships a Part 11 IQ protocol and completed OQ and PQ reports at no extra cost.[^gg-pricing] It publishes an entry price of $12,000 a year, with implementation quoted separately, and Vendr reports a median of $43,989 a year.[^gg-pricing][^gg-vendr] For a Reliance user, the trade-offs are scale and scope. Users report it getting cumbersome with several product lines and weak for BOMs and change requests, and there are no pharma GMP modules.[^gg-reddit-worth][^gg-reddit-cost][^gg-pricing] Complaint handling stops at a task to prepare the FDA report, with no eMDR submission.[^gg-complaints]

### You make devices and drugs and want EU hosting: Qualio

[Qualio](/reviews/qualio/) runs deviations, OOS and pharmacovigilance in the same platform as its device design controls.[^q-pharma] Those design controls support up to three requirement levels with verification and validation test cases and ISO 14971 risk.[^q-dc] Its public subprocessor list names one web-application host: AWS in Dublin.[^q-subprocessors] Its data processing addendum lists an active ISO 27001 certification.[^q-dpa] G2 reviewers rate its ease of use 9.0/10 (663 ratings, Sep 2026).[^q-g2] Like Octave, Qualio doesn't publish prices, and every plan carries an implementation fee.[^q-pricing] Complaints run on configurable event templates, and we found no FDA MDR or EU vigilance reporting, so you'd give up Reliance's eMDR form.[^q-forms][^q-qmsr]

### You build hardware with complex BOMs and suppliers: Arena QMS

[Arena QMS](/reviews/arena-qms/) ties CAPAs, complaints and training to the same parts and change orders that engineering uses.[^ar-quality] Suppliers get their own licences and role-based access to the product record.[^ar-supply] Arena runs a separate European instance on AWS, covered by PTC's public data processing addendum and sub-processor list.[^ar-security-platform][^ar-ptc-dpa][^ar-ptc-sub] You can request a 10-day free trial without a credit card.[^ar-pricing][^ar-trial] Pricing is quote-only, and the validation package costs extra.[^ar-pricing] There's no risk register or FMEA object, so you'd lose Reliance's FMEA app.[^ar-g2] After the contract ends, a one-time data export is charged as professional services.[^ar-terms]

### You want a similar scope with a known price ceiling: Ideagen Quality Management

[Ideagen Quality Management](/reviews/ideagen-quality/) is also a multi-industry QMS, with CAPA, audit, supplier and document workflows documented in a public help centre.[^id-help-api] Its Medical Devices FastStart pack costs "below £20,000 per year" and includes 5 admin and 10 user licences, configuration and two days of training.[^id-faststart] Its UK G-Cloud price list (October 2025) publishes per-user rates and implementation at £1,330 a day.[^id-gcloud-price] Data is hosted on AWS in the UK, the EEA or elsewhere as agreed in the order form, and Ideagen holds ISO 27001:2022.[^id-gcloud][^id-dpa][^id-certs] It shares Reliance's main gap: design controls, design history files and ISO 14971 risk management appear only on the FastStart sales page, not in the help centre.[^id-faststart][^id-help-api] Unless the order form says otherwise, the standard agreement runs for 24 months and renews automatically, and yearly fee rises have no cap.[^id-mssa]

### You build software as a medical device: Matrix Requirements or Ketryx

[Matrix Requirements](/reviews/matrix-requirements/) documents configurable traceability rules and links issues to Jira, GitHub, GitLab and Azure DevOps.[^mx-trace][^mx-git] Its template package covers IEC 62304.[^mx-sxmd] Prices are published: three Matrix Req users plus ten Matrix Quality licences come to about $21,800 a year.[^mx-list2026][^mx-quality] A full QMS means two products on different platforms, Matrix Req and the Salesforce-based Matrix Quality.[^mx-quality]

[Ketryx](/reviews/ketryx/) syncs requirements, software items, tests and risks from Jira, Git and test tools and generates the design history file from them.[^kx-dhf] Its own platform is certified to IEC 62304, ISO 13485 and ISO 14971.[^kx-certs] Ketryx describes itself as a bridge between engineering tools and your existing eQMS, so it could sit next to Reliance rather than replace it.[^kx-meddev] It offers a 30-day self-serve trial, but paid plans are contact-sales, and we found no documented supplier, internal audit or production record workflows.[^kx-signup][^kx-pricing][^kx-llms]

## When to stay with ETQ Reliance

Reliance has real strengths for large manufacturers, and switching an eQMS takes work:

- **Configurability.** More than 40 configurable applications, changed with visual tools or a scripting language for advanced logic.[^etq-reliance][^etq-eqms]
- **Scale.** Trane runs it for more than 9,000 users across 52 sites.[^etq-apps]
- **Complaints with eMDR.** The life sciences set adds complaint handling with an electronic form for FDA medical device reports, and Octave says it integrates with an electronic submission gateway.[^etq-lsc] None of the alternatives above documents eMDR submission.
- **Supplier collaboration.** Suppliers can log in as external users, and Illumina's suppliers answer audits and SCARs directly.[^etq-nxg][^etq-illumina]
- **Devices and drugs.** Its GxP apps include deviations and OOS lab investigations.[^etq-apps]
- **Exit.** You can ask for your data in an industry-standard format at no charge within 30 days of the contract ending.[^etq-msa]

If design controls are your only gap, consider whether a dedicated design-control tool next to Reliance would be less disruptive than a full switch.
